Montefiore School Health Program
About the Program
Antonia Pantoja Preparatory Academy is fortunate to be served by the Montefiore School Health Program. The program features a full staff including, psychologist, doctor, nurse, etc. dedicated to the health of your child in the form of a school clinic. To enroll your child in the program, parents must fill out an enrollment form. The clinic provides care for students while they are in school. Students still keep their primary doctors, however, if your child becomes sick, needs to take or have medicine administered, or needs counselling while in school, The needed care will be performed by a medical provider. There is no cost for using this program, but students need to be enrolled to receive direct medical care, physicals, mental health services, immunizations etc.

The School-Based Health Center (SBHC) is situated in the school building and is operated by Montefiore Medical Center. All enrolled students can access medical, behavioral health, and dental health services during the school operating hours at the health center within the school. Enrollment will allow you to register your child/children electronically into the school-based health. Given current federal recommendations for COVID prevention and safety, the e-consent enrollment process will minimize your in-person contact and increase access for parents to complete the enrollment form at your convenience. Services are offered both in-person and via telehealth (Using Montefiore F1rst), which provides the opportunity for students to access health care in school and from home.

We kindly ask that you follow the steps below to complete the registration form:
- Click here (Spanish version) to access the enrollment form
- Complete all required fields and provide e-signatures where asked
- Save a copy of the completed form to your device
- Save the document using your child’s first and last name (i.e. John Smith)
- Email the completed e-consent to SouthBronxCampus@Montefiore.org. If available, please attach insurance cards and vaccination records if available.
